Preservation databases

I am interested in finding out:  who has preservation databases; what
kind of information the institution or person is collecting; what kind
of software package(s) they are using; Is the database linked to other
areas of collections control such as exhibits or accessions, etc. Could
people send a sample print-out to me either over Email, through direct
mail or via fax?
