When I want to insert track breaks into CD-DA versions of interviews, I
just break the piece up into 5 minute long tracks, remove the silences that
Nero automatically places between the tracks, and then burn the disc.

In this case (being a commercial release and all) such an approach seems
somewhat--I don't know--inelegant.  Is there any other way this can be
done? Is there a way to break up the audio so that the program time will
not be reset at every break? How the heck do books-on-CD pull this stuff off?

The answer is: yes and no. Yes, there is a mechanism to index within tracks, but no you don't want to use it since it is not implemented on all players and it is used so rarely that many people who have the capability are unfamiliar with it.

The usual display on a CD device is time-within-track, but many offer an
option of time since the start of the first track, time remaining in this
track or on the disc, and possibly other options. FWIW, LaserDiscs show
elapsed time of the disc by default along with chapter number; DVD-Video is
simlar except that it can hold multiple titles. Thus, on a CD one locates a
moment by track and time; on LD, by time alone; on DVD by title and time.
